  • How does Topiramate work?

    Topiramate works by stabilizing the electrical activity in the brain. It acts on several systems, including inhibiting carbonic anhydrase, modulating voltage-gated sodium and calcium channels, and activating GABA-A and AMPA/kainate receptors, contributing to its weight loss effects. This is thought to lead to feelings of fullness and decreased caloric intake. 

  • How long do I need to take Topiramate before I see weight loss?

    To get the best results with medication, you need to consistently take the medication, eat a healthy diet, and engage in physical activity. It’s also important to have regular check-ups with your healthcare provider to track your progress and adjust your treatment if needed. In clinical trials, weight loss was noted after 12-16 weeks of treatment with Topiramate.

  • Does Topiramate have any known side effects?

    The most common side effects of Topiramate include: 

    • somnolence (feeling tired)
    • difficulty concentrating
    • tingling sensations
    • dizziness
    • difficulty sleeping
    • depression
    Although rare, serious side effects may occur including:
  • metabolic acidosis
  • kidney stones
  • decreased sweating leading to hyperthermia
  • acute angle-closure glaucoma

  • What precautions should I take when using Topiramate?

    Topiramate is safe for most people. However, you SHOULD NOT take Topiramate if you have:

    • kidney or liver disease
    • history of glaucoma
    • history of kidney stones
    • serious depression. if you have history of depression and/or suicide, reach out to your care provider before starting Topiramate. 
    Do NOT get pregnant or breastfeed while taking this medication, as studies show that it can be associated with birth defects. Additionally, Topiramate may cause your birth control to be less effective. We recommend touching base with your provider to discuss alternatives and/or strategies to prevent pregnancy while using this.

  •  Can I use Topiramate with other medications?

    Topiramate is safe to use with most medications. However, we recommend NOT starting Topiramate if you are using other anti-seizure medications or a carbonic anhydrase inhibitor such as acetazolamide, zonisamide, dorzolamide etc. If you have a history of kidney stones, we recommend touching base with your primary care provider prior to starting Topiramate. 

  • Who is a good candidate to use Topiramate?

    Because the weight loss with Topiramate is relatively modest, we recommend using this as a supplementary treatment to enhance the weight loss effects of other medications. Topiramate can be particularly helpful for those with chronic pain or migraine headaches, as it can also alleviate these symptoms. 

  • How much does the program cost?

    The medical consult fee is a one time charge of $39. Patients will also be charged a monthly provider support fee of $59/month. If you are prescribed oral medication, you may be able to use insurance. If you use cash, your treatment plan will cost $60-105/month depending on your medications (plans with Bupropion HCL SR and Naltrexone HCL are $60; plans with Bupropion HCL SR, Naltrexone HCL, and Topiramate start at $90; plans with Bupropion HCL SR, Naltrexone HCL, Metformin HCL ER, and Topiramate start at $105)..
